When you are looking for work, it's a good idea to practice with blank job application forms before completing a formal application for employment.

Information Requested on Blank Job Application Forms

The job application form you are given will likely ask for the following kinds of information:

Name and Contact Information

The first thing the prospective employer will want to know is your full legal name and your address. If you prefer to be known by your second name or a nickname, you can add this information to the form. You will also be asked for your phone number.

Work Eligibility and Criminal Record

There will also be a section on the form asking you to confirm that you are legally eligible to work in the country. You will also be asked whether you have a criminal record. Some employers will only ask about felony convictions within the previous five years, but others will want to know about any prior convictions.

Position and Availability

You will need to explain what job title or type of position you are interested in applying for. The employer will also want to know what days of the week and hours you are available to work, as well as when you would be available to start.

Education

Part of filling in a job application form involves providing some information about your education. You will be asked where you went to school, whether you completed the degree or diploma program, what your major was, and what year you graduated.

Work History

Blank job application forms will also have a section for you to fill in describing your previous work experience. Information you will be asked to provide includes:

  • Name and address of previous employer
  • Job title
  • Date of hire
  • Date you left
  • Salary/rate per hour
  • Reason for leaving

Once you have finished filling out the form, you will be asked to sign it as an acknowledgment that the information you have provided is correct and complete. It's very important to provide accurate information. Providing false or misleading information on your application form may be considered grounds for dismissal if the employer finds out later on.

How to Fill in a Job Application Form

  • Be sure to provide all the information the prospective employer has asked for. If you don't have the answer at hand, such as a date, take the form home to complete it and bring it back to the employer later.
  • Print your answers neatly and use a blue or black pen to do so.
  • Put your most recent education first when you get to that part of your application form, then work backwards in time. College, university or a trade school would go first, and then you can include where you went to high school.
  • Your job history should be listed with your current or most recent position first. Then work backward in time and list each one in reverse chronological order.
  • Take a few minutes to reread your application to make sure that is it complete and accurate. You also want to check for spelling or grammar errors.
  • Include references if you are asked to. Not all of them need to be from people you have worked with. You can also include professors, teachers or coaches if you feel that they can give a potential employer some idea about what kind of employee you would be.
  • If the form has a spot for indicating whether the prospective employer can contact your current one and you don't want them to do so, don't hesitate to say, "No." It may lead to an awkward situation if your current employer doesn't know that you are looking for work and a prospective employer calls for a reference.
